adb: win32: improve console output on Windows 10
Use Windows 10's improved virtual terminal emulation to handle more
terminal sequences, to fix console resize artifacts, and to fix issues
with the last column.
`top' and `more' work well now.
Use a sensible default TERM value of xterm-256color.
Test: manual on Windows 10
